We follow the privacy practices described in this notice while it is in effect. At Artful Orthodontics, protecting your family's personal information is something we take seriously and are committed to every day.

For example, we may need to provide information to your insurance company to receive pre-approval for a planned procedure or to verify your coverage and benefits.

We may charge a reasonable fee for copies. You may also request records in electronic format when the information is maintained electronically.

Right to an accounting of disclosures
